maybe you didn't survive the wounds that were caused by the fire? was your health in the last red bar?
Well i can remember some flamethrower guy came and unleashed his rage, i could shoot him and jump into a pool, and the fire stopped. my health reached the orange bar after the attack.

I think if your burnt by a flamethrower, you can be burned on the surface of water. But if you go underwater, it will stop, but if you go back up, it will burn again.
